Medical Social Consultant - PHS

Position Summary This position works under general supervision of the Supervisor of Medical Social Services and provides medical case management and coordination of care services. In addition, this position supervises Medical Social Associates and Assistants and consults with care team in the development and organization of specific aspects of social services needed for positive health outcomes.
