Overview Job Duties Provides coordination of care to patients under the direction of the CHFM Medical Directors. Assists with specific in-basket electronic communication, clinical operations, and counsels’ patients regarding their health. Virtually asses’ patient care and obtains appropriate input as needed to determine by NPP/Physician under the direction of the Clinic Medical Director.
